CONFERENCE REGISTRATION

Instructions:   Online registration will be available until midnight on Friday, April 3, 2009. Please note the following registration fees and deadline dates:
  • Early Registration Fee: $300.00 (until Friday, April 3, 2009)
  • Late/On-site Registration Fee: $350.00 (after Friday, April 3, 2009)
  • Student Registration Fee: $150.00 (until Friday, April 3, 2009). Must show student ID at the conference
*NOTE: To be assessed the early registration fee, your registration payment must be received by April 10, 2009. If your payment is received AFTER that date, you will be automatically assessed the late registration fee of $350.00.

Once you have registered, you will be directed to a link that provides you with an e-mail confirmation. Please consider this e-mail your receipt (if you paid by credit card) or your invoice (if you paid by check). Be sure to retain this e-mail for your records.
Participant Login System: Once you have registered, you may re-enter the Web site to update your contact information by going to the Participant Login page web site.

Please be sure to keep a record of your Registration ID number (located at the top of the confirmation e-mail you should receive after registering) to re-enter the site. You will need both the Registration ID number and your e-mail address to modify your registration form.
Payment Instructions:   The following registration payments will be accepted for the conference:
  • American Express
  • Visa
  • Master Card
  • Checks*
  • Purchase Order**
*Checks should be made payable to National Association of Chronic Disease Directors (Federal Tax ID #731328414). Please mail a copy of your e-mail confirmation and the check to:

**If you plan to pay by Purchase Order and your check has not arrived prior to the conference please be prepared to pay on-site and/or bring your purchase order request with you.

Cancellation Policy: All cancellations MUST be received IN WRITING (i.e., via fax or email) no later than Friday, April 10, 2009. If written cancellation notices are received by that date, a full refund, less a $25 processing fee, will be granted. Refunds cannot be given for cancellations received after April 10, 2009, or for "No Show".
